Einfluss der laparoskopischen bilateralen Zerviko- (LACESA) und Vaginosakropexie (LAVASA) auf die Misch- und Dranginkontinenz der Frau

Zusammenfassung

Genitalprolaps und Harninkontinenz sind weltweit verbreitet. Wegen ihrer Komplexität stellen sie nach wie vor eine große medizinische Herausforderung dar. Auf Basis der Integraltheorie von Petros und Ulmsten wurden die laparotomische Zerviko- bzw. Vaginosakropexie (CESA bzw. VASA) als operative Therapie bei Drang- und Mischinkontinenz entwickelt. Nun wurde die laparotomische Form erstmals auf ein minimalinvasives laparoskopisches Verfahren (LACESA bzw. LAVASA) umgestellt und standardisiert. Wir berichten über 154 Patientinnen, die zwischen März 2013 und Juli 2017 operiert wurden. Die bilaterale apikale Korrektur eines Deszensus (Zervix- oder Scheidenstumpf) in Zusammenhang mit einer Harninkontinenz durch PVDF-Bandersatz (PVDF: Polyvinylidenfluorid) der Uterosakralligamente (USL) in minimalinvasiver laparoskopischer Technik sowie Ergänzung durch eine transobturatorische Bandeinlage (TOT) erreichte eine Kontinenzrate von 92 %. Die klassische unilaterale Fixation am Promontorium (z. B. Sakrokolpopexie) schützt nicht vor einer De-novo-Harninkontinenz. Im Gegensatz dazu wurde bei LACESA und LAVASA ein „Kontinenzeffekt“ beobachtet. Es wird angenommen, dass die Bilateralität, die definierten Fixationsstellen (Verlauf der Uterosakralligamente zwischen Zervix/Scheidenstumpf und Os sacrum auf Höhe von S1/S2) und die einheitliche Länge sowie das Material (PVDF) der USL-Band-Ersatzstrukturen eine entscheidende Rolle spielen.
