Der Einsatz von „platelet-rich plasma“ (PRP) in der Reproduktionsmedizin bei endometrialen Pathologien – eine narrative Übersichtsarbeit zum aktuellen Wissensstand

Zusammenfassung

„Platelet-rich plasma“ (PRP), im Deutschen auch plättchen- oder thrombozytenreiches Plasma genannt, ist per Definition ein Blutplasmaprodukt mit erhöhten Thrombozytenkonzentrationen, welches durch Plasmapherese aus meist autologem Vollblut gewonnen wird. Dem PRP wird ein erhöhtes regeneratives und immunmodulatorisches Potenzial zugeschrieben. Dies leitet sich aus dem hohen Gehalt an unterschiedlichen Wachstumsfaktoren und vielen weiteren Substanzen ab, die unter anderem durch die Thrombozyten freigesetzt werden können. Die Variabilität in den PRP-Protokollen stellt eine enorme Herausforderung für die Vergleichbarkeit der Studienergebnisse dar. Sie trägt auch maßgeblich dazu bei, dass oft keine ausreichende Evidenz für einen therapeutischen Einsatz von PRP vorhanden ist. Mittlerweile hat auch die Reproduktionsmedizin das PRP für sich entdeckt. PRP wird dort unter anderem bei insuffizientem Endometriumaufbau, Asherman-Syndrom und rezidivierendem Implantationsversagen unklarer Genese eingesetzt. Diese Übersichtsarbeit soll die mögliche Wirkungsweise des PRP in den genannten Einsatzbereichen sowie die gegenwärtige Evidenz darlegen. Dabei wird auch ausführlich auf die Problematiken und Herausforderungen eines PRP-Einsatzes im reproduktionsmedizinischen Bereich eingegangen.
