Clozapintherapie bei therapieresistenter Schizophrenie

Zusammenfassung

Schizophrenie ist eine mithilfe von Antipsychotika behandelbare psychiatrische Störung. Die Wirksamkeit der Antipsychotika bessert jedoch nur einen Teil der Symptome und dies auch nicht bei allen Betroffenen. Bei therapieresistenter Schizophrenie gilt Clozapin weltweit als Goldstandard. Es wird aber wegen Sicherheitsbedenken (Agranulozytose, Clozapin-Myokarditis) und schlechter Verträglichkeit (epileptische Anfälle, Speichelfluss, Gewichtszunahme, Sedierung etc.) selten und fast immer erst viele Jahre nach schizophrener Erstmanifestation angewandt. Patienten mit therapieresistenter Schizophrenie werden zu selten auf Clozapin eingestellt. Aus Angst der Behandler vor bedrohlichen Nebenwirkungen wird die Clozapintherapie zudem häufig wieder umgestellt.
