Choosing Wisely – Gegen Überversorgung im Gesundheitswesen – Aktivitäten aus Deutschland und Österreich am Beispiel der Geriatrie

Zusammenfassung

Die American Board of Internal Medicine (ABIM) Foundation initiierte 2012 die Choosing Wisely Kampagne in den USA. Ziel der Kampagne ist die Kommunikation zwischen Ärzten und Patienten sowie ihren Angehörigen zu fördern über diagnostische und therapeutische Maßnahmen, die keinen Zusatznutzen bieten und mit Schaden für Patienten assoziiert sind. Die American Geriatrics Society (AGS) und die American Medical Directors Association – The Society of Post-Acute and Long-Term Medicine (AMDA) haben jeweils 10 Empfehlungen für ältere Patienten formuliert. Weitere Gesellschaften haben Empfehlungen publiziert, die insbesondere für ältere Patienten Relevanz haben. Die internationale Verbreitung hat dazu geführt, dass 2014 ein International Roundtable gegründet wurde. In Deutschland hat 2015 die Deutsche Gesellschaft für Medizin (DGIM) mit ihren Schwerpunktgesellschaften eine Initiative begonnen, die in Erweiterung der amerikanischen Initiative sowohl Überversorgung wie auch Unterversorgung thematisieren will. Die Deutsche Gesellschaft für Geriatrie (DGG) arbeitet in dieser Initiative aktiv mit, ebenso in der Kommission der Arbeitsgemeinschaft der Wissenschaftlich-Medizinischen Fachgesellschaften (AWMF), die ein Methodenpapier zur Entwicklung von Empfehlungen erarbeitet hat. Die Deutsche Gesellschaft für Allgemeinmedizin und Familienmedizin (DEGAM) adressiert in einer neuen Leitlinie dieses Thema ebenfalls und auch in Österreich entwickeln sich erste Aktivitäten. Eine transparente, nachvollziehbare, strukturierte und evidenzbasierte Vorgehensweise erscheint angeraten, um methodische Schwächen der amerikanischen Initiative zukünftig zu vermeiden. Ein wichtiger Unterschied der Vorgehensweise im Vergleich zu den USA liegt in diesem methodischen Ansatz und in der Zielsetzung, ergänzend zu Punkten der Überversorgung auch solche zur Unter- und Fehlversorgung zu identifizieren.
