Calcimimetika – Update 2022

Zusammenfassung

Anstiege von sowohl Parathormon (PTH) als auch „fibroblast growth factor 23“ (FGF-23) sind frühe Zeichen einer „chronic kidney disease – mineral and bone disorder“ (CKD-MBD). Für beide Hormone wurden Assoziationen mit der Patientensterblichkeit berichtet. Für FGF-23 gibt es auch experimentelle Hinweise für eine direkte Kardiotoxizität, insbesondere Linksventrikelhypertrophie (LVH).
Die Einführung von Calcimimetika hat die Behandlungsoptionen der CKD-MBD deutlich erweitert. Calcimimetika binden an den Calcium-sensing-Rezeptor (CaSR) der Nebenschilddrüsen und aktivieren diesen. Sie simulieren eine Bindung von Calcium an den CaSR, daher auch der Name Calcimimetikum. Calcimimetika (Cinacalcet, Etelcalcetid) senken sowohl PTH- als auch FGF-23-Spiegel bei CKD-Patienten. Insbesondere die Senkung von FGF-23 könnte sich günstig auf die kardiovaskuläre Morbidität und Mortalität von CKD-Patienten auswirken. Diese Vermutung wird mittels Post-hoc-Analysen und prospektiven Untersuchungen bezüglich des Verlaufs der LVH unter Calcimimetikatherapie untermauert. Bedauerlicher Weise liegen jedoch keine Daten vor, die belegen, dass eine Verbesserung der Surrogat-Endpunkte auch tatsächlich zu einem relevanten Nutzen für Patienten wie Reduktion von kardiovaskulären Events oder verbessertem Patientenüberleben führt.
