Besonderheiten der multiplen Sklerose im Kindes- und Jugendalter

Zusammenfassung

Die multiple Sklerose (MS), eine chronisch-entzündliche Erkrankung, wird in 3–5 % der Fälle vor dem 16. Lebensjahr diagnostiziert. Obwohl sich die MS des Kindes- und Jugendalters kaum von der Erkrankung des Erwachsenenalters unterscheidet, gibt es doch einige Besonderheiten, die den Verlauf der Erkrankung im Kindesalter beeinflussen können. Kinder und Jugendliche mit MS weisen zu 95–98 % einen primär schubhaften Verlauf mit Komplettremission auf. Im Rahmen eines Schubs können sich Sehstörungen, Sensibilitätsstörungen, Koordinationsstörungen (Stand­ und Gangataxie), Symptome durch Hirnstammbeteiligung und auch motorische Defizite bis hin zu Lähmungen zeigen. Bei der Hälfte der Patienten treten bei Erstmanifestation monosymptomatisch Symptome einer Neuritis nervi optici, einer Hirnstammbeteiligung sowie motorischer Defizite auf. Im weiteren Krankheitsverlauf können auch mehrere Symptome auftreten. In den ersten 2 Erkrankungsjahren zeigen Kinder und Jugendliche mit MS eine höhere Schubrate. Das Auftreten von kognitiven Defiziten, Fatique und Depression ist häufig und kann erhebliche psychosoziale Konsequenzen haben. Im Langzeitverlauf kann es zu einer Zunahme des kognitiven Defizits und Abnahme des Intelligenzniveaus kommen. Eine bleibende Behinderung und ein Übergang in eine sekundär chronisch progrediente Verlaufsform treten im Vergleich zu der Erwachsenen-MS trotz anfänglich langsamer Krankheitsprogression 10 Jahre früher ein.
