Zusammenfassung

Sowohl klinisch als auch laborchemisch liegt bei unserem Patienten eine CASPR-2-AK-positive autoimmune Enzephalitis vor. Dafür spricht das subakute Auftreten von kognitiven Defiziten, begleitet von Stimmungsschwankungen und Schlafstörungen, komplex-fokalen Anfällen und Kurzzeitgedächtnisstörungen. Der entzündliche Liquorbefund mit positiven oligoklonalen Banden und der wiederholte Nachweis von CASPR-2-AK im Liquor und Serum unterstützen die Diagnose. Derzeit ergeben sich keine Hinweise auf eine paraneoplastische Genese. Das Komplex-Syndrom autoimmune Enzephalitis und Neuromyotonie (Morvan-Syndrom), welches mit CASPR-2-AK assoziiert sein kann, konnte in diesem Fall nicht bestätigt werden. Interessanterweise bietet der Patient zusätzlich Symptome eines zerebellären Syndroms, die klinisch einer episodischen Ataxie Typ I ähneln. Die EA1 ist eine autosomal-dominant vererbte Erkrankung, die durch Mutationen in einem neuronalen Kaliumkanal verursacht wird. Bei unserem Patienten konnten Antikörper gegen CASPR-2, ein Protein, das für eine normale Funktion dieses Kaliumkanals wichtig ist, gefunden werden. Es ist durchaus möglich, dass die CASPR-2-AK in diesem Fall die Funktion des Kaliumkanals so verändern, dass hier klinisch ein EA1 ähnliches zerebelläres Syndrom entstanden ist. Somit könnte es sich hier um eine erworbene episodische Ataxie Typ I handeln.
