Assessment-Tools zur Identifizierung eines Palliativbedarfes bei Herzinsuffizienzpatienten in der allgemeinmedizinischen Praxis

Zusammenfassung

Die palliative Betreuung von Patientinnen mit nicht-onkologischen, chronischen Erkrankungen liegt größtenteils in den Händen der Hausärzte. Derzeit verwenden praktische Ärztinnen aber kaum standardisierte Fragebögen, um den Bedarf zu ermitteln. Vielmehr ist es eine intuitive, individuelle Entscheidung, zu welchem Zeitpunkt und in welchem Ausmaß eine Palliativbetreuung zu Hause stattfindet. Anhand des Beispiels eines Patienten mit chronischer Herzinsuffizienz wird der Frage nachgegangen, ob und mit welchen Assessment-Tools der Bedarf einer (spezialisierten) Palliativbetreuung in der Allgemeinmedizin erkannt werden kann.
