Anästhesiologische Perspektive zum perioperativen Management bei kontinuierlicher subkutaner Insulininfusion (CSII): Stoppen oder fortführen?

Zusammenfassung

Zielsetzung

Der perioperative Umgang mit Insulinpumpen (CSII) ist uneinheitlich. Ziele dieser Arbeit waren die Literatursuche zum perioperativen Management bei CSII, Analyse der gefundenen Evidenz und Ableitungen für die perioperative klinische Praxis.

Methodik

Die systematische Literaturrecherche erfolgte in PubMed und Clinical Research Network.

Ergebnisse und Diskussion

Es wurden 11 Publikationen zu CSII in der perioperativen Medizin identifiziert. Demnach gewährleistet die CSII in bestimmten klinischen Situationen glykämische Stabilität. CSII könnte bei geplanten kleinen, nicht-herzchirurgischen Operationen mit Fasten von nur einer Mahlzeit belassen werden. Internationale Guidelines gehen nicht auf CSII perioperativ ein. Es gibt keine bundesweiten Registerdaten zur CSII; der Einsatz dürfte um 18 % liegen. Von den Ergebnissen wurden ein krankenhausinternes Protokoll für das perioperative Management mit CSII und eine Patienteninformation abgeleitet.

Schlussfolgerung

Aufgrund wachsender Evidenz für ein verbessertes Patientenoutcome durch die perioperative Fortführung der CSII sollten Kenntnisse und Erfahrungen über die intensivierte Insulintherapie interdisziplinär vertieft und Patienteninformationen verfügbar gemacht werden.
