Zusammenfassung

Transgenderpersonen rücken immer stärker in die Aufmerksamkeit der Öffentlichkeit. Das ist nicht der Fall für die Transgendermedizin, die (noch) ein stiefmütterliches Dasein fristet. Nur wenige Zentren weltweit beschäftigen sich wissenschaftlich mit dem Phänomen der Genderinkongruenz und seiner Behandlung. Die Zahl der Transgenderpersonen ist in den letzten Jahren explosionsartig gewachsen, diese Personen suchen verstärkt adäquate medizinische Versorgung, sowohl im Rahmen der transition aber auch danach für die lebenslange notwendige Betreuung. Mediziner sind auf diese Patientengruppe nicht vorbereitet. Weder universitäre Curricula noch die Ausbildungskataloge der verschiedenen Spezialismen berücksichtigen die spezifische Fragen und Bedürfnissen von Trans*Personen. Als besonders schwierig empfinden Mediziner die Tatsache, dass die Ziele der Behandlung jenseits des fachspezifischen Horizonts liegen. Es geht darum, einer Person zu ermöglichen, ein selbstbestimmtes, genderkongruentes, weitestgehend Genderdysphorie-freies und gesundes Leben zu führen. Diese Perspektive ist Somatikern nicht geläufig. Sie bedeutet, dass die fachspezifische Behandlung in einen größeren Kontext gestellt wird und als Teilstück eines holistischen Konzepts zu verstehen ist. Auch das Dilemma der subkutanen Transphobie in der Medizin wird angesprochen oder das zunehmend auftretende Phänomen der sogenannte non-binary identities: Personen, die sich selbst jenseits der traditionellen Mann-Frau Dichotomie verorten. Wie können wir als Mediziner damit umgehen? Dieser Beitrag zieht eine Spur entlang der Bruchlinien zwischen Medizin und Gesellschaft, wie sie im Umgang mit Trans*Personen sichtbar werden. So sehr wir als MedizinerIn auch versuchen, auf die inhaltlichen medizinischen Probleme zu fokussieren, so unmöglich ist dies im Umgang mit Transgenderpersonen. Es erweist sich als hilfreich, die Transgendermedizin und im Besonderen die Hormonbehandlung im Rahmen eines transdisziplinären Teams auszuüben und Kenntnis über die sozialen Realitäten von Transgenderpersonen zu erwerben.
