Zusammenfassung

Zerebrale Infarkte mit primär psychiatrischer Symptomatik sind seltene und ungewöhnliche Ereignisse. Wir berichten über einen 50-jährigen Patienten mit einer plötzlich aufgetretenen paranoiden Psychose und Symptomen einer wahnhaften Missidentifikation. Als Ursache stellten sich beidseitige subakute Kleinhirninfarkte heraus. Auslösend scheinen das vorbestehende vaskuläre Hochrisikoprofil sowie eine extreme psychosoziale Belastung im Vorfeld gewesen zu sein. Unter hochdosierter antipsychotischer Medikation kam es zu einer völligen Remission der psychopathologischen Symptomatik.
