Bewegung und Training bei rheumatoider Arthritis

Zusammenfassung

Die rheumatoide Arthritis (RA) ist eine chronische, systemische, entzündliche Autoimmunerkrankung der Gelenke mit einer Reihe von Begleit- und Folgeerscheinungen, wie z. B. Muskelatrophie bis hin zur rheumatoiden Kachexie, Osteoporose, verminderte aerobe Kapazität, erhöhte Inzidenz und Progression von Herz-Kreislauf-Erkrankungen. Dies hat auffallende Ähnlichkeit mit den Folgen körperlicher Inaktivität (Detrainingseffekte). Während die RA selbst durch einschlägige medikamentöse Therapie behandelt wird, steht derzeit außer Frage, dass sich praktisch alle Folgeerscheinungen der RA durch regelmäßige Bewegung im Alltag (Erhöhung des Energieumsatzes durch Muskelaktivität) und/oder Training (anabol wirksame Bewegungsreize) günstig beeinflussen lassen. Besonders betont wird, dass die günstigen Bewegungs- und Trainingseffekte lebenslang durch kontinuierliche Bewegung aufrechterhalten werden können. Bewegung wird differenziert in Alltagsbewegung, Üben sowie Training von Muskelkraft und aerober Ausdauer. Die Methodik, insbesondere des Krafttrainings und des aeroben Ausdauertrainings, wird in diesem Beitrag eingehend besprochen.
