Zusammenfassung

Hintergrund

Gastrostomien werden bei Kindern offen, laparoskopisch oder endoskopisch angelegt. Unabhängig von der Methode treten Komplikationen relativ häufig auf. In einem 5-Jahres-Rückblick stellen wir Komplikationen und Probleme dieses scheinbaren Routineeingriffs dar und schlagen für die Anlage einer Gastrostomie bei Kindern ein möglichst sicheres Verfahren vor.

Material und Methoden

An unserer Abteilung durchgeführte Gastrostomien aus dem Zeitraum von 2009 bis 2013 wurden retrospektiv analysiert. Indikation, Technik und Komplikationen wurden verglichen.

Ergebnisse

Bei 60 Patienten waren insgesamt 62 Gastrostomien angelegt worden. Es wurden 7 Major-und 9 Minorkomplikationen verzeichnet. Wobei Majorkomplikationen ausschließlich bei der perkutanen endoskopischen Gastrostomie (PEG) und bei der offenen Gastrostomie auftraten. In der Gruppe der laparoskopischen Gastrostomien fanden sich lediglich Minorkomplikationen. Eingriffsassoziierte Todesfälle ereigneten sich nicht.

Diskussion

Die Gesamtmorbidität bei der pädiatrischen Gastrostomie ist hoch. Schwerwiegende Komplikationen treten bei der PEG gehäuft auf; gefürchtet ist die gastrokolische Fistel. Die offene Anlage einer Gastrostomie ist in seltenen Fällen weiterhin indiziert. Mit Einführung der laparoskopischen Gastrostomie können schwerwiegende Komplikationen vermieden werden.

Schlussfolgerung

Als sicheres Verfahren empfehlen wir die Durchführung einer laparoskopischen Gastrostomie bei Kindern.
