HIV-Testung an der Allgemeinbevölkerung


Internationale Empfehlungen und reale Risken für HIV-Infektionen für Gesundheitsberufe

Zusammenfassung

In Österreich werden deutlich mehr HIV-Tests durchführt als in anderen Europäischen Ländern. Es ist zu vermuten, dass dies auf unfokussierte präoperative Testung von Patienten im Spital zurückzuführen ist. Die vorliegende Übersichtsarbeit befasste sich mit internationalen Empfehlungen zu HIV-Screening an der Allgemeinbevölkerung und zu realen Ansteckungsrisiken für Gesundheitspersonal. Eine universelle HIV-Testung wird international nur empfohlen, wenn die undiagnostizierte Prävalenz von HIV in der Bevölkerung > 0,1 % bzw. die diagnostizierte Prävalenz > 0,2 % ausmacht. HIV stellt für Gesundheitsberufe ein kleines, aber reales Gesundheitsrisiko dar. Weltweit wurden seit Beginn der Zählungen (90er Jahre) bis 2002 106 gesicherte und 238 mögliche berufsbedingte HIV-Infektionen gezählt: in Österreich wurden in 15 Jahren (1996–2011) 4 berufsbedingte HIV-Infektionen gemeldet. Keine überzeugende Evidenz liegt vor, dass das Wissen um den Serostatus eines/r PatientIn zu verändertem Verhalten des Gesundheitspersonals führt. Da Österreich mit einer HIV Prävalenz von 0,1 % als Niedrigprävalenz-Land gilt, wird eine stärker fokussierte Teststrategie empfohlen.
