Die Wertigkeit der diagnostischen und interventionellen Koloproktoskopie aus der Sicht des Viszeralchirurgen

Zusammenfassung

Die klinische Gastroenterologie bietet ein weites Feld der interdisziplinären Kooperation. Der vorliegende Artikel soll die enorme Bedeutung der diagnostischen und interventionellen Koloproktoskopie für den Viszeralchirurgen aufzeigen. Die Autoren versuchen, die Wertigkeit der Endoskopie anhand zahlreicher Beispiele aus dem chirurgischen Alltag zu belegen. Neben der klassischen „Adenom-Karzinom-Sequenz“, wird ein erst in den letzten Jahren erkannter, neuer Weg der Karzinogenese bei den sessilen, serratierten Adenomen beschrieben. Da sich dabei kleine Adenome auf dem „serratierten Weg“ schneller zu Karzinomen entwickeln können, ist derzeit die vollständige Polypektomie aller entdeckter Polypen zu empfehlen. Die interventionelle Koloskopie wird wahrscheinlich in Zukunft, aufgrund der schonenderen, patientenfreundlicheren, minimal-invasiven Technik traditionelle chirurgische Therapien ergänzen oder gar ersetzen. Hierbei muss sich der Viszeralchirurg seiner alten Tradition und der damit verbundenen Kompetenz in der Therapie von Kolorektalkarzinomen bewusst sein und sich aktiv in die neue interdisziplinäre Entwicklung der Koloproktoskopie einbringen.
