Metabolische Chirurgie oder konservative Maßnahmen zur Behandlung adipöser Typ 2-Diabetiker?

Zusammenfassung

Nach bariatrischen Operationen zeigte sich vielfach neben der Gewichtsabnahme auch eine günstige Beeinflussung von Begleiterkrankungen wie Störungen des Glukose- oder Fettstoffwechsels, weshalb für operative Maßnahmen zur gezielten Beeinflussung metabolischer Störungen, vor allem des Diabetes mellitus Typ 2, der Begriff „metabolische Chirurgie“ geprägt wurde. Die komplexen Mechanismen, die deren metabolischen Wirkungen zugrunde liegen, sind noch unklar. Energierestriktion und (im Verlauf) Gewichtsverlust sowie dessen Erhalt scheinen entscheidende Faktoren zu sein.
Bei den in der metabolischen Chirurgie eingesetzten, der bariatrischen Chirurgie entlehnten Verfahren werden die operativen Risiken in qualifizierten Zentren als relativ niedrig angegeben. Da es sich um elektive Eingriffe handelt, sind die Mortalität und Morbidität, die Häufigkeit von Therapieversagen und Zweiteingriffen besonders zu beachten; ebenso die vielfältigen, irreversiblen, nicht selten schwerwiegenden und auch potentiell lebensbedrohlichen Folgen solcher Operationen, die lebenslang eine konsequente, interdisziplinäre Nachsorge und nötigenfalls Therapie (insbesondere Substitution bei Malassimilation) erforderlich machen. Über langfristige Auswirkungen der operativ veränderten Anatomie und Funktion der Verdauungsorgane ist wenig bekannt; bis zur Manifestation klinischer Probleme können viele Jahre vergehen.
Bei adipösen Diabetikern (BMI ≥ 35 kg/m2) sollte daher erst bei Erfolglosigkeit einer individuell abgestimmten multimodalen konservativen Therapie die Option der metabolischen Chirurgie (im Sinne einer „Ultima Ratio“) in Betracht gezogen werden – möglichst im Rahmen von geeigneten Studien. Ein darüber hinausgehender Einsatz ist wegen fehlender adäquater Langzeit- und Vergleichsdaten derzeit nicht zu begründen.
