Zusammenfassung

Tumorschmerzen und Schmerzen bei Nichttumorerkrankungen können mit Schmerzmechanismen wie einer peripheren bzw. zentralen Sensibilisierung oder Deafferenzierung einhergehen. Die Abklärung erlaubt über klinische Zeichen wie eine Allodynie oder Hyperalgesie indirekte Rückschlüsse auf die zugrunde liegenden Mechanismen. Basis einer Tumorschmerztherapie nach dem Stufenschema der Weltgesundheitsorganisation (WHO) sind Nichtopioide. Bei sehr starken Tumorschmerzen kann die Schmerztherapie direkt von Stufe 1 auf Stufe 3 eskaliert werden. Opioide sind in der Behandlung von Tumorschmerzen gut wirksam, auch bei neuropathischer Komponente, die in bis zu 40 % der Fälle als Mixed-pain-Syndrom auftreten kann. Koanalgetika stellen eine wertvolle Therapieergänzung zur besseren Schmerzkontrolle dar, mit ihnen können auch Komorbiditäten wie eine Angststörung, Depression oder Schlafstörung behandelt werden. Bei Einschränkung der Leber- und/oder Nierenfunktion ist die Dosierung vieler Arzneistoffe anzupassen. Bei der Therapie multimorbider oder schwerkranker Patienten mit Opioiden und Antidepressiva/Antikonvulsiva sind zahlreiche mögliche pharmakodynamische und pharmakokinetische Wechselwirkungen zu beachten.
