Steuerung der NOAK-Gabe bei invasiven oder operativen Interventionen

Wann bzw. wie pausieren und wann wieder starten?

Zusammenfassung

Bei vielen Patienten, die unter einer Therapie mit oralen Antikoagulazien stehen, sind perkutane oder chirurgische Eingriffe notwendig. Bei den Vitamin-K-Antagonisten (VKA) gibt es Empfehlungen, wie prä- bzw. postinterventionell vorgegangen werden soll.
Das Management der neuen oralen Antikoagulanzien (NOAK) sollte eigentlich einfacher sein – dennoch müssen einige Zusammenhänge beachtet werden. Dies liegt an den unterschiedlichen pharmakokinetischen Profilen der Substanzen Dabigatran, Rivaroxaban, Apixaban und Edoxaban.
Beim periprozeduralen Management von Patienten, die mit NOAK behandelt werden, muss das Risiko von Blutungen und Thromboembolien beachtet werden. Die NOAK haben einen pharmakokinetischen Vorteil in Bezug auf einen schnellen Wirkungseintritt und die rasche Elimination aus dem Körper über Leber und Nieren. Bei eingeschränkter Nierenfunktion kann sich die Halbwertszeit der NOAK aber deutlich verlängern.
Operationen unter NOAK können bei geringem/minimalem Risiko für Blutungen zu Beginn des nächsten Dosierungsintervalls geplant werden, sodass lediglich auf 2–3 Gaben verzichtet werden muss. Bei Operationen mit moderatem und hohem Blutungsrisiko sollte vor dem Eingriff eine NOAK-Pause von 24–48 h erfolgen, um ein entsprechendes Abklingen der Wirkspiegel zu ermöglichen. Bei Patienten mit niedrigem/mittlerem Risiko für Thromboembolien ist kein Bridging notwendig, wenn die „ungeschützte“ Zeit (NOAK-Pause) weniger als 5–7 Tage beträgt. Bei Patienten mit hohem Risiko für Thromboembolien müssen individuelle Patienten- und chirurgische Faktoren (lange Dauer bis zur Hämostase) berücksichtigt werden, bevor die Entscheidung zum Bridging bzw. zum Verzicht hierauf getroffen werden kann. Ob auf Bridging bei NOAK generell verzichtet werden kann, muss in randomisierte Studien oder Registern für das periprozedurale Management von NOAK-Patienten geklärt werden.
