Chirurgische Aspekte der Peritonealdialyse

Zusammenfassung

Die Peritonealdialyse (PD) findet seit den 70er Jahren einen breiten klinischen Einsatz. Klinische Daten zeigen einen Überlebensvorteil der PD gegenüber der Hämodialyse in den ersten 2 Jahren. Trotzdem werden in Österreich derzeit nur etwa 10 % der dialysepflichtigen Patienten mittels PD betreut. Für eine erfolgreiche Langzeitfunktion ist nicht nur die Compliance der Patienten sondern auch eine sorgfältige Katheterimplantation notwendig.
Neben der klassischen „offenen“ Operationsmethode über eine Minilaparotomie wurden in den letzten Jahren vor allem minimalinvasive Implantationstechniken weiterentwickelt. Als Pro-Argument für die laparoskopische Katheterimplantation wird die Möglichkeit für Zusatzeingriffe und die optimale Platzierung der Katheterspitze genannt. Zu den häufigsten Chirurgie-assoziierten Komplikationen zählen Leaks oder Kathetermigration, viel seltener treten Infektionen auf. Es konnte jedoch in mehreren Studien kein Vorteil für minimalinvasive Techniken bezüglich Komplikationen gefunden werden.
Diese Arbeit soll einen Überblick über die derzeit gängigen Implantationstechniken geben.
