Synthetische Tränen – Ein Schritt näher der menschlichen Tränenflüssigkeit

Zusammenfassung

Hintergrund

Bis heute ist es noch nicht gelungen, die menschliche Tränenflüssigkeit mit all ihren Komponenten synthetisch herzustellen. Dies ist auf den komplexen Aufbau der Tränenflüssigkeit zurück zu führen. Tränen bestehen nicht nur aus Wasser und Salz, sondern enthalten vielmehr auch unzählige Proteine, Lipide, Muzine, Antioxidantien sowie ein eigenes Puffersystem. Tränenersatzmittel, die unter anderem zur Therapie des Trockenen Auges eingesetzt werden, enthalten meist wenig bis kaum Bestandteile der natürlichen Tränen. Für einen optimierten Tränenersatz bedürfte es des Zusatzes mehrerer Komponenten der natürlichen Tränenflüssigkeit. Eine dieser Komponenten betrifft auch die Inhaltsstoffe mit antioxidativer Schutzwirkung. Die vorliegende Arbeit untersucht Substanzen, die antioxidativ wirken und eine Schutzwirkung vor UV-Licht und Ozon aufweisen und in solchen Präparaten zur Anwendung kommen könnten. Desweiteren werden mögliche weitere physiologische Komponenten eines optimierten Tränenersatzes beleuchtet.

Material und Methoden

Mithilfe von Viskositätsbestimmungen wurden verschiedene Substanzen, die in der Literatur als Radikalfänger bezeichnet werden, auf eine mögliche Schutzwirkung gegenüber Umwelteinflüssen wie UV-Licht und Ozon untersucht. Dazu wurden alle Substanzen in genau definierten Konzentrationen in 0,25 % Hyaluronsäure gelöst, unter konstanten Untersuchungsbedingungen mit UV-B Licht bestrahlt oder Ozon bedampft und anschließend deren Viskosität mithilfe eines KPG-Ubbelohde Viskosimeters bestimmt. UV-Licht und Ozon führen zu einer Depolymerisierung und damit verbundenen Viskositätsverminderung von Hyaluronsäure. Dies wird auf die Entstehung freier Radikale durch UV-Licht und Ozon zurückgeführt. Eine geringere Viskositätsverminderung bzw. ein Gleichbleiben der Ausgangsviskosität trotz Bestrahlung oder Bedampfen wurde als Schutzwirkung der zugesetzten Substanz interpretiert. Bezüglich anderer Tränenfilmhauptkomponenten wurde eine Recherche der aktuellen Literatur zusammengefasst.

Resultate

Insgesamt wurden elf Substanzen auf ihre Wirkung gegenüber UV-Licht und Ozon untersucht: Euphrasia und Mannit konnten Hyaluronsäure signifikant vor einer Depolymerisirung durch UV-Licht und Ozon schützen (p < 0,05). Harnsäure und Melatonin konnten gegenüber UV-Licht eine signifikante Wirkung erzielen (p < 0,05), nicht aber gegen Ozon. Arginin, Curcumin, Fructose, Harnstoff, Lysin, Spermidin, Taurin konnten keine Verhinderung der Depolymerisierung bewirken. Von den Tränenfilmhauptkomponenten haben Natriumchlorid, Glucose und Bikarbonat als Inhaltsstoffe in Tränen Schlüsselfunktionen für die Homöostase der Augenoberfläche.

Schlussfolgerung

Für einen optimierten Tränenersatz ist der Einsatz von Bestandteilen der natürlichen Tränenflüssigkeit wie Natriumchlorid, Glucose und Bikarbonat als Hauptpuffersubstanz zu bevorzugen. Im Hinblick auf die antioxidative Wirkung können Substanzen, wie Euphrasia und Mannit zum Einsatz kommen. Beide Substanzen können die sowohl in den Tränen wie auch in einigen Tränenersatzmitteln enthaltene Hyaluronsäure vor einer Depolymerisation durch UV-Licht und Ozon schützen und damit potentiell ihre Verweildauer und Schmierwirkung an der Augenoberfläche verlängern.
