Qualitätsindikatoren zur Beurteilung der Lebensqualität von Krebspatienten

Zusammenfassung

Hintergrund

Neben patientenberichteten Outcomeparametern werden international auch aus klinischen Daten Qualitätsindikatoren (QI) zur Beurteilung der Versorgungsqualität von Tumorpatienten am Lebensende erhoben.

Fragestellung

Relevanz klinischer QI für Lungenkrebspatienten und Durchführbarkeit der Erhebung im klinischen Kontext sowie über Krebsregister.

Vorgehen

Literaturrecherche und Erhebung an vier deutschen Zentren.

Ergebnisse

Die beobachtete große Streubreite von Patienten mit tumorspezifischer Therapie in den letzten 14 Lebenstagen ist durch die Art der Erhebung (prospektiv oder retrospektiv, Erhebung im Rahmen von Studien oder über Register- bzw. Versicherungsdaten) und durch unterschiedliche Einschränkungen der Grundgesamtheit weitestgehend erklärt worden: verschiedene Tumorentitäten, Ausschluss von Patienten jünger als 65 Jahre (z. B. bei Nutzung der SEER-Datenbank) oder auch den Ausschluss der kurz nach Diagnose versterbenden Patienten. Daten zur Tumortherapie in den letzten 14 Lebenstagen konnten mit geringem Aufwand sowohl klinisch als auch über das regionale Tumorzentrum erhoben werden. Die Erfassung des Hospiz- und Palliativkontakts und der Dokumentation einer Vorausverfügung zum Lebensende war deutlich aufwändiger (Akteneinsicht).

Schlussfolgerungen

Die Erhebung der tumorspezifischen Therapie in den letzten 14 Lebenstagen ermöglicht die Reflexion des onkologischen Handelns bei finalkranken Tumorpatienten. Gleiche Grundgesamtheiten für Zähler und Nenner ermöglichen auch einen länder- und zentrumsübergreifenden Vergleich.
