In Österreich leben ca. 72.700 Menschen in Langzeitpflegeeinrichtungen (Statistik Austria 2013). Typischerweise treffen bei diesen in der Regel alten Menschen multiple chronische Erkrankungen zusammen, erschwerend hinzu kommen oft funktionelle und kognitive Defizite (Onder 2011). Die Medikamentenverordnung bei diesen Patienten ist eine besondere Herausforderung und bedarf besonderer Sorgfalt im Bezug auf intendierten Nutzen und möglichen Schaden. Aufgrund der Prävalenz von multiplen chronischen Erkrankungen ist Polypharmazie ein häufiges Phänomen (Onder 2012), als Folge ist das Risiko für unerwünschte Arzneimittelereignisse erhöht. …
