Neue internationale Referenzwerte für die Spirometrie – Implikation für die Praxis anhand einer vergleichenden Analyse eines pädiatrischen Patientenkollektivs

Zusammenfassung

Hintergrund

Die neuen Lungenfunktionsreferenzwerte der Global Lung Initiative (GLI) sind von den meisten Fachgesellschaften empfohlen. Ziel dieser Arbeit war es anhand eines pädiatrischen Patientenkollektivs aus Deutschland und Österreich FEV1- und FEV1/FVC-Werte nach alten und neuen Referenzwerten vergleichend zu analysieren.

Ergebnisse

Bei 215 kaukasischen Kindern (5–17 Jahre) waren FEV1-Werte nach GLI signifikant niedriger als nach Zapletal (Median 96,9 % d. S. (87,3–105,2) versus 100,6 % d. S. (Quartile 91,5–111,4), p = 0,000), die Differenz betrug im Median 4,9 %, die Spannweite − 12,9–27,5 % d. S. Die Differenzwerte korrelierten signifikant mit dem Alter der Probanden und der Höhe der FEV1 % d. S. nach Zapletal (p = 0,000). Von 70 Patienten mit cystischer Fibrose hatten 17 (24 %) FEV1-Werte < 80 % d. S. (Zapletal), bei 25 (36 %) Patienten waren die FEV1-Werte nach GLI pathologisch (Z-Score < 1645). 3/145 Asthmatikern (2 %) hatten FEV1-Werte < 80 % d. S. (Zapletal), 7/145 (5 %) Z-Scores < 1645 (GLI).

Schlussfolgerung

Bei pädiatrischen Patienten mit Asthma und CF bestehen erhebliche Unterschiede zwischen GLI-und Zapletal Referenzwerten.
