Die Boston-Keratoprothese: eine Übersicht auf der Basis eigener Erfahrungen

Zusammenfassung

Ist die Erfolgsprognose für eine Hornhauttransplantation schlecht und/oder ist eine Keratoplastik wiederholt gescheitert, bieten Keratoprothesen die einzige Chance zur visuellen Rehabilitierung. Die Boston-Keratoprothese, welche seit 50 Jahren kontinuierlich fortentwickelt worden ist, wird heute bei weitem am häufigsten angewandt, mit in den letzten Jahren auch international erheblich zunehmenden Implantatzahlen.
Die aktuelle Konstruktions-Version und die eigene Implantationstechnik werden ausführlich dargestellt; dabei werden die eigenen Kriterien zur Wahl des Durchmessers der Träger-Hornhaut und der Rückplatte vorgestellt, sowie die Überlegungen ausgeführt, die bezüglich der Durchführung zusätzlicher operativer Eingriffe zur Behebung von Komorbiditäten in gleicher Sitzung zu beachten sind.
Die eigenen bisherigen Ergebnisse an insgesamt 29 Fällen zwischen 2009 und 2014 (Nachbeobachtung 1 Monat bis 5 Jahre) werden bezüglich des Visus, der Retention und der Komplikationen vorgestellt: Der Visus beträgt zwischen intakter Lichtprojektion und 0,8 und wird im Wesentlichen durch die Komorbiditäten bestimmt. Die primäre Retentionsrate beträgt 90 % (26/29), die Retentionsrate nach Revision 97 % (28/29).
Haupt-Komplikationen umfaßten zystoides Makulaödem (10/29), Sekundärglaukom (6/29), sterile Vitritis und Glaskörpertrübungen (4/29), retroprothetische Membran (2/29) und Netzhautabhebung (3/29).
In der Diskussion werden die aus den eigenen Erfahrungen und aus dem Schrifttum gezogenen Rückschlüsse zur Indikationsstellung, zu den Voraussetzungen für gute Erfolgsprognose und für die Nachbehandlung ausführlich erläutert.
