Auswirkungen stationärer Aufnahmen auf das Rauchverhalten psychiatrischer und somatischer Patienten

Zusammenfassung

Hintergrund

Im Rahmen zweier Stichtagsuntersuchungen an psychiatrischen und somatischen Stationen eines Schwerpunktkrankenhauses wurde der Einfluss stationärer Aufnahmen auf das Rauchverhalten von Patienten untersucht.

Methoden

Befragung von Patienten an der psychiatrischen Abteilung und somatischen Stationen an 2 Stichtagen mit einem Fragebogen zur Erfassung demografischer Daten und von Rauchgewohnheiten inklusive des Fagerström-Tests. Primäres Ziel war die Feststellung einer etwaigen Änderung im Rauchverhalten, sekundäres Ziel Unterschiede zwischen psychiatrischen und somatischen Patienten bzw. innerhalb der psychiatrischen Diagnosegruppen im Rauchverhalten zu evaluieren. Da die Daten nicht normalverteilt waren, wurden für die statistische Auswertung nichtparametrische Tests und visuelles Klassifizieren zur Herstellung von Gruppenvergleichen verwendet.

Ergebnisse

Bei psychiatrischen Patienten hatte ein Anteil von 26,4 % im Gefolge des stationären Aufenthaltes mehr geraucht bzw. wieder oder neu zu rauchen begonnen. Im Gegensatz dazu traf dies für keinen einzigen der somatischen Patienten zu. In zwei Altersgruppen konnten signifikante Unterschiede zwischen somatischen und psychiatrischen Patienten im geänderten Rauchverhalten während des stationären Aufenthalts und hinsichtlich der Schwere der Abhängigkeit festgestellt werden. Unter den psychiatrischen Patienten wiesen Patienten mit schizophrenen Störungen in 85,7 %, Patienten mit affektiven Störungen in 44,4 % eine schwere Nikotinabhängigkeit auf.

Schlussfolgerungen

Infolge der geringen Fallzahl und der erschwerten Vergleichbarkeit der Gruppe somatischer und psychiatrischer Patienten sind weitere Untersuchungen erforderlich, um einerseits den Einfluss stationärer Aufnahmen auf das Rauchverhalten zu evaluieren und um andererseits Maßnahmen zu entwickeln, die geeignet sind den Tabakkonsum in Gesundheitseinrichtungen einzudämmen.
