Zusammenfassung

Der folgende Fallbericht beschäftigt sich mit dem Kinderwunsch männlicher Patienten unter einer Biologikatherapie und den damit einhergehenden möglichen Risiken. Studien zeigen, dass eine TNF-Blocker-Therapie bei männlichen Patienten während der Zeugung nicht mit einem erhöhten Risiko von Missbildungen oder Schwangerschaftskomplikationen verbunden ist. Eine TNF-Blocker-Therapie scheint die männliche Fruchtbarkeit nicht zu verringern. Allerdings gibt es nur sehr wenige Daten über eine Vaterschaft unter Biologikatherapie. In der Literatur sind 60 Fälle beschrieben. Der Verlauf wurde nur bei 28 Schwangerschaften dokumentiert. Mit dem Einfluss von TNF-Blockern auf die Spermatogenese setzen sich 5 Studien auseinander. Eine beschreibt einen positiven Effekt von TNF-Blockern auf Motilität und Vitalität von Spermien, wahrscheinlich durch eine Reduktion der Krankheitsaktivität. Im Gegenzug wurde eine verminderte Spermienqualität bei Patienten ohne TNF-Blocker-Therapie herausgefunden. Kaum eine nationale Gesellschaft gibt dazu eine Empfehlung. Die Entscheidung über das Absetzen oder Fortsetzen einer TNF-Blocker-Therapie bei geplanter Vaterschaft muss mit den betroffenen Männern und Frauen nach Kenntnis und Aufklärung über die vorhandene Datenlage im Einzelfall entschieden werden.
