Ein Fallbericht über die Problematik der Ernährung am Lebensende eines onkologischen HNO-Patienten

Zusammenfassung

Das Auftreten von Kachexie am Lebensende eines Krebspatienten ist eine häufige Komplikation maligner Erkrankungen. In den letzten Jahren versuchte man neue Definitionen, Diagnosekriterien und Klassifikationssysteme für Kachexie (Lancet Oncol 12:489–495, 2011) zu entwickeln, um den klinischen Alltag leichter zu gestalten. Die therapeutischen Möglichkeiten bleiben beschränkt, der Einsatz von parenteraler Ernährung (PE) wird kontrovers diskutiert und kann bei Patienten mit unheilbarer Erkrankung selbst bei unzureichender oraler oder enteraler Ernährung aufgrund des veränderten Stoffwechsels nicht generell empfohlen werden (Clin Nutr 28:445–454, 2009). Bei der Behandlung eines kachektischen Patienten mit HNO-Tumor wurden wir mit unterschiedlichen Schwierigkeiten konfrontiert.
