Tools zur Therapieoptimierung bei multimorbiden geriatrischen Patienten am Lebensende – Hilfe oder „nur“ Denkanstoß?

Zusammenfassung

Die optimale Pharmakotherapie für multimorbide geriatrische Patienten am Lebensende ist eine große Herausforderung für alle Behandler. Das Einschätzen der Prognose kann im Vergleich zu Krebserkrankungen viel schwieriger sein, ist aber essentiell, um eine Therapie im Gesamtkontext von Erkrankung, Komorbiditäten, Allgemeinzustand, Lebenserwartung und Therapiezielen zu erstellen.
Da diese Patienten ein erhöhtes Risiko für unerwünschte Arzneimittelwirkungen haben, sind inadäquate Medikationen zu vermeiden. Die Evidenz für zahlreiche Medikamente bezüglich Wirkung und Risiken ist gering. Einige Tools können dennoch helfen, die Pharmakotherapie für diese vulnerable Patientengruppe zu verbessern.
