Bei der Einführung der Laparoskopie war die Reduktion des Zugangstraumas der entscheidende Vorteil gegenüber der Laparotomie. Auch gegenwärtig ist das Bestreben in der Laparoskopie, durch einen verbesserten Zugang das Bauchdeckentrauma weiter zu minimieren. Ermöglicht wird dies durch die Industrie, die kleine Trokare mit einem Durchmesser von 2 bis 3 mm sowie dünneres laparoskopisches Instrumentarium (wie z. B. Fasszangen oder Scheren), das durch diese Trokare passt, anbietet ([1]; Abb. 1). Auch verspricht man sich von der Single-incision-laparoscopic-surgery(SILS)-Technik durch die Reduktion der Zugänge von mehreren auf einen eine Minimierung des Zugangstraumas [2, 3, 4]. Neben dem Zugangstrauma sind weitere Kriterien für die Patienten wichtig wie
  • intra- und postoperative Komplikationen,
  • postoperative Schmerzen,
  • die Länge des Krankenhausaufenthalts,
  • postoperatives Aussehen und
  • kurzzeitige Rehabilitation.
