Zur Wertigkeit der Erregerbestimmung bei odontogenen Infektionen in der klinischen Routine

Zusammenfassung

Dentogene Abszesse sind typischerweise polymikrobiell mit aktuell steigenden Resistenzquoten gegenüber einigen Antibiotika. Das Ziel der Studie bestand darin, den Wert von Erreger- und Resistenzanalysen unter klinischen Routinebedingungen zu analysieren.
In einer prospektiven Pilotstudie wurden Abstriche dentogener Abszesse unter Routinebedingungen entnommen. Die Resultate zum Erregerspektrum und zur Resistenzsituation wurden mit denen einer früheren Untersuchung verglichen.
In die Studie flossen 11 Männer, 5 Frauen und 3 Kinder ein (Alter 2–86 Jahre). Es konnten 38 Erreger isoliert werden, wobei ein fast ausgewogenes Verhältnis aerober zu anaeroben Spezies (1:1) vorlag. Die Resistenzquoten waren für Penicillin G nahezu unverändert, während beim Clindamycin im aeroben Bereich eine zweifache, im anaeroben Bereich eine gut 10-fache Erhöhung beobachtet wurde.
Eine detaillierte Übersicht über Resistenzzunahmen sowohl im aeroben als auch im anaeroben Bereich erfordert weitere prospektive Untersuchungen mit normierten Entnahme- und Transportbedingungen.
